At University Otolaryngology, patient care is our top priority.  Thank you for choosing us.

Your First Visit

Welcome to University Otolaryngology!  On your first visit to our office please bring:

  • Your insurance cards
  • A list of medications you are taking
  • Your referral form (if required)
  • Copies of all previous test results (if applicable)
  • Actual films for MRIs, CT Scans or X-Rays
  • Completed registration forms (which can be downloaded below)
Patients under 18 must be accompanied by a parent or legal guardian.

Cancellations

If you must cancel your appointment, please call us at least 24 hours in advance. This allows us to give your appointment time to another patient in need of our services. 

 

Emergencies

If you have a medical emergency after office hours:

  • Call 911 immediately for medical emergencies
  • For emergencies that are not life-threatening you may call (401) 274-2300 and the on call physician will be contacted

Insurances Accepted

We participate with most major medical plans, including state, federal and private insurance plans.  Please contact our office for details.

The following forms may be downloaded to your computer, printed and completed prior to your office visit.  Doing so will expedite your appointment.  Our staff will be happy to answer any questions you have regarding these forms on the day of our visit.

Follow Up Visits

Welcome back!  On your follow up visit you will be asked to present your insurance card for verification purposes.

If more than a year has elapsed since your last visit, you will be asked to complete an updated medical history form, which you may download below.

If your condition requires surgery we will bill your insurance company.

Each insurance plan has different levels of coverage.  Some pay in full for covered services, while others have deductibles, co-insurance payments and office co-pays.  Patients are ultimately responsible for payment of services and should be familiar with their insurance carrier's benefits and coverages. 

If you require service and are uninsured, please contact our office at (401) 274-2300. 

If you have any questions regarding our fees, you may discuss them with our billing office. 

Our office fees are based on the services necessary to diagnose and/or treat your medical condition.

We participate in most major medical insurance plans and you'll be told if we participate you your particular health plan prior to your visit.

We accept cash, personal checks, VISA, MasterCard and American Express.

 

Your insurance company will be billed for CT-Scans and other diagnostic tests performed on the day of your visit.  You are responsible for payment of any amount not covered by your insurance.  All copays are due at the time of your visit.
